John,
Though I don't wish to be flamed as a "Bill Gates
fan," I do happen to use Microsoft Word--as it does
just about everything I could ever imagine needing...
I don't know if you remember the "good old days," a
mere decade ago, when too many of us professional
writers were still banging it out on manual
typewriters (something "macho" about the resistance in
the keys), using "white-out" and "cutting and pasting:
excerpts onto fresh sheets (so that after a coupla
draft runs one sheet could assume very thick
proportions...)
On the top "toolbar" you click "insert" with the mouse
and go to "index and tables" (several version
avbailable--scholarly, classic, etc.); your options
are "table of authorities," or "table of contents,"
etc. Piece of cake. And if you need it as an "HTML"
version, simply save it as such...
best,
Imbas
